These policies govern all training programs offered by OM SPA, LLC, including but not limited to in-person training, live online instruction, and independent self-study formats. These policies apply to all enrolled students and third-party payors and outline the terms of registration, participation, and certification.

OM SPA, LLC operates as a private trade education provider and is not affiliated with or accredited by any government institution. Federal financial aid (e.g., FAFSA or Title IV) is not accepted.
1. REGISTRATION & PAYMENT TERMS
1.1 A non-refundable registration fee is required to secure enrollment.
1.2 Credit or debit card payments are subject to a 3% processing fee.
1.3 Payments made via Zelle to [email protected] are not subject to processing fees.
1.4 Full tuition is due no later than 14 calendar days prior to the course start date.
1.5 Enrollment is confirmed only upon receipt of the signed agreement and payment of the registration fee.
1.6 All payments made are final, non-refundable, and non-transferable.
1.7 OM SPA, LLC may automatically charge any outstanding balance using the payment method on file if tuition is not paid by the due date.
1.8 Students utilizing third-party financing (e.g., Affirm, Klarna, PayPal Pay-in-4) are solely responsible for applying independently.
1.9 OM SPA, LLC does not apply for financing on behalf of students and does not guarantee approval.
1.10 Financing contracts are strictly between the student and the third-party provider.
1.11 It is the student’s responsibility to understand all applicable repayment terms, fees, and interest.
1.12 OM SPA, LLC bears no responsibility for incomplete or denied financing applications and is not required to reserve seats.
1.13 Tuition paid via third-party financing is considered fully paid, regardless of future issues with repayment to the lender.
1.14 Tuition payments via third-party financing remain non-refundable and non-transferable.
1.15 Tuition payments made through financing platforms may not be disputed, reversed, or charged back.
1.16 OM SPA, LLC assumes no liability for issues involving third-party financing providers.
2. THIRD-PARTY PAYMENT TERMS
2.1 If a third party (Payor) funds the training, the Payor agrees to all stated terms.
2.2 The Payor also acknowledges the no-refund and no-chargeback policy.
2.3 OM SPA, LLC is not responsible for disputes between students and Payors regarding expectations, outcomes, or satisfaction.
2.4 Completion and success of the course are determined by the student’s engagement, effort, and compliance.
2.5 OM SPA, LLC reserves the right to charge the payment method on file if a scheduled payment is missed or not completed on time. If tuition is paid in installments, subsequent payments are expected to be submitted manually by the student or designated payor. If payment is not received by the specified deadline, OM SPA, LLC may initiate automatic billing. By submitting payment, the individual confirms authorization to use the specified card or account and agrees not to initiate chargebacks or reversals.
3. STUDENT ELIGIBILITY & MEDICAL REQUIREMENTS
3.1 All students must be at least 18 years of age.
3.2 A professional license is recommended but not mandatory. Students must meet all physical and medical requirements.
3.3 Students must have adequate manual dexterity and 20/20 vision (corrected or uncorrected).
3.4 Students must be in appropriate physical and mental health to perform cosmetic procedures.
3.5 Students must not be colorblind and must accurately distinguish directions.
3.6 Valid, readable photo identification is required.
3.7 Students must be fluent in English or Polish; if not, a qualified translator must be provided.
3.8 OM SPA, LLC is not responsible for misunderstandings resulting from language barriers.
3.9 It is the student’s responsibility to research local licensing and legal eligibility for cosmetic tattooing.
4. COURSE MATERIALS & SUPPLIES
4.1 Digital course materials are provided in advance and must be reviewed by the student prior to class. Students are expected to print or bring digital access to these materials.
4.2 Pre-study is mandatory for all in-person and live online courses. Failure to review materials beforehand may impact the student’s ability to participate fully.
4.3 All required in-class practice supplies will be provided by OM SPA, LLC.
4.4 A basic starter kit is included upon successful completion of the course. Additional or advanced kits may be purchased separately.
4.5 Once skin is opened during a live model procedure, any items left unprotected on the workstation — such as pigments, needles, or disposables — will be deemed contaminated and discarded. The student is responsible for the replacement cost of any such items.
5. DRESS CODE & FACILITY
5.1 Students are required to wear full-length black scrubs (top and bottom), fully closed-toe anti-slip shoes, tie back long hair, and keep nails trimmed short.
5.2 Personal Protective Equipment (PPE), including gloves, aprons, masks, and protective barriers, must be worn during procedures and will be supplied by OM SPA, LLC.
5.3 All training sessions are conducted in a licensed and insured facility compliant with state safety requirements.
6. ATTENDANCE & PERFORMANCE
6.1 Full attendance is required for successful certification. Students must complete all assignments, evaluations, and hands-on components.
6.2 Tardiness is not accommodated in group settings. Missed content will not be repeated. Students must review missed material independently.
6.3 For private 1:1 training, instructors may provide additional guidance, but only within the originally scheduled hours.
6.4 Students must complete any live model procedures they initiate, even if it extends beyond the scheduled training time. Abandoning a procedure or asking the trainer to complete it is strictly prohibited.
6.5 Chronic lateness or absences — regardless of personal circumstances — may require a private make-up session at an additional fee. These sessions are not guaranteed and are based on trainer availability.
6.6 If a student cannot attend their scheduled in-person training, OM SPA, LLC may, at its discretion, offer access to a self-study course or allow attendance at a future group training for a rebooking fee, if space is available.
6.7 Students requesting private make-up training must pay full 1:1 training rates. All seats are non-refundable and first-come, first-served.
7. MODELS (IN-PERSON COURSES ONLY)
7.1 OM SPA, LLC provides demonstration models when available, but students are strongly encouraged to bring their own pre-screened models.
7.2 Models recruited by students must meet eligibility criteria and be pre-qualified in accordance with OM SPA’s model guidelines provided during training.
7.3 OM SPA, LLC invests in marketing efforts to help support model acquisition, but cannot guarantee model availability or compliance.
7.4 If a scheduled model cancels or is disqualified, the student will continue practice on synthetic materials. Model participation enhances training but is not required to complete the course.
8. CANCELLATIONS, FORCE MAJEURE & MAKE-UP
8.1 All payments are final. OM SPA, LLC does not issue refunds for any reason, including cancellations, no-shows, or missed training days.
8.2 In-person course transfers may be granted with a minimum of 60 days’ written notice sent to [email protected].
8.3 Make-up for missed in-person sessions due to illness may be approved at OM SPA’s discretion for an additional fee and based on trainer availability.
8.4 In the event of force majeure (e.g., weather, health emergencies, government orders), OM SPA will offer alternatives such as rescheduling or converting to online formats.
8.5 If a training is postponed due to trainer emergency, OM SPA, LLC will offer one of the following: rescheduled dates, access to online training, or a partial refund (maximum 50%) if no alternative is possible within 30 days. OM SPA, LLC is not liable for indirect expenses (e.g., travel, lodging).
8.6 OM SPA, LLC assumes no responsibility for student-incurred travel costs or missed work due to training interruptions. Flexible travel plans are strongly recommended.
9. ONLINE COURSES
9.1 Live online sessions may be conducted via Zoom, Facebook Group, WhatsApp, or similar platforms. Students must ensure a quiet workspace and have the necessary technical setup (camera, microphone, high-speed internet).
9.2 OM SPA, LLC is not liable for student-side technical issues. Replays may be provided only if sessions are recorded and student requests access within 7 days. Links may be time-limited and additional fees may apply.
9.3 If no recording is available, students must rely on manuals and course materials. Optional paid recap sessions may be requested.
9.4 OM SPA is not obligated to record or store sessions unless agreed in writing.
9.5 Remote-only students are not required to complete Bloodborne Pathogen Certification.
9.6 Practice kits are shipped prior to class. All online practice is completed using artificial materials.
9.7 Optional in-person practice sessions may be available for a fee and based on availability.
9.8 Self-study students receive 10 days of guided digital access and lifetime access to applicable pre-recorded materials.
9.9 Private mentorship and feedback options may be purchased separately.
9.10 OM SPA, LLC does not offer refunds or free rescheduling for missed online sessions.
9.11 Certificates for online formats are awarded at the trainer’s discretion based on engagement and completion of required submissions.
10.CERTIFICATION
10.1 Certificates are issued only after full completion of coursework and submission of required documentation.
10.2 In-person students receive certificates marked “With Hands-On Application.”
10.3 Live online students receive certificates marked “Live Online Training” upon attendance or submission of completed assignments (e.g., pattern designs, practice skin photos).
10.4 Self-study students receive certificates marked “Self-Study/Online” upon completion and review of all required exercises.
10.5 Feedback is included in live formats. Self-study students may purchase mentorship separately.
11. LEGAL & LIABILITY
11.1 OM SPA, LLC is not liable for injuries, reactions, or outcomes resulting from student actions, errors, or use of third-party products during or after training.
11.2 Students and Payors release OM SPA, LLC and affiliates from liability for any personal, emotional, or physical injury occurring during or after the course.
11.3 Students agree not to initiate chargebacks. Unauthorized reversals are subject to a $250 administrative fee and potential legal action.
11.4 All legal matters will be governed by the laws of Illinois. Disputes must be handled within Cook County jurisdiction.
11.5 In multilingual contracts, the English version is the legally binding reference.
11.6 Electronic and digital signatures hold full legal weight under federal and state law.
11.7 All course content, documents, and recordings are proprietary and may not be reused or redistributed without written consent.
11.8 Students may not offer training or open a PMU academy within a 20-mile radius of OM SPA, LLC for 18 months post-completion unless otherwise approved in writing.
11.9 OM SPA makes no employment or earnings guarantees. Success depends on the student’s individual effort.
11.10 Courses provide foundational skills only. Mastery requires continued education, practice, and dedication.
11.11 Group course participants may be required to work independently at times. One-on-one supervision is not guaranteed. Supplemental support may be available for an additional fee.
12. INTELLECTUAL PROPERTY & MEDIA
12.1 All course content is the intellectual property of OM SPA, LLC, and may not be copied, shared, or used for teaching without written approval.
12.2 Unauthorized recording of any portion of training is prohibited.
12.3 OM SPA, LLC may record or photograph classes for documentation or marketing purposes.
12.4 Participation in training constitutes consent to be photographed and recorded unless a written opt-out notice is submitted in advance to [email protected].
These policies reflect OM SPA, LLC’s commitment to high training standards, operational transparency, and safety. Enrolled students and financial sponsors are responsible for full compliance.
